THE LOUISIANA MEDICAL BOARD OFFERS AN APPLICATION FOR THIS PATHWAY.
The Louisiana State Board of Medical Examiners has begun issuing licenses to International Medical Graduates (IMGs) that hold a degree from an international medical program that is “substantially similar” to the education or training required to practice in Louisiana.
To qualify for a Foreign Physicians licensed here are some of the following requirements that must be met:
- Fully Trained Foreign Physician Application and non-refundable 382$ fee
- All time accounted for from medical school graduation to the present
- Ink signed Statement of Legal Name form (This will be mailed with the paper application)
- Notarized Third Party Authorization form (This will be mailed with the paper application and must be notarized in person, not online)
- Ink signed Oath or Affirmation form (This will be mailed with the paper application)
- Notarized typed, ink signed affidavit for Oath or Affirmation form “Yes” answer(s) (if applicable. Must be mailed with the paper application)
- Verification of International medical licenses and translations (Good standing medical license in his\her country)
- CBC Results (background check)
- Physicians must have an offer for employment at a rural area hospital
- Statement from each facility where you practiced in your home country verifying that you had no quality or other problems with staff
- Verification of acceptable Medical School (verified via World Directory of Medical Schools (Medical degree must be translated in English)
- Verification of USMLE Exam 1-3 scores
- Verification of Residency (Royal College or ACGME-I)
- Verification of Specialty Board Certification (if applicable)
- Possesses basic fluency in the English language (ECFMG certificate)
Proof of Legal Status (Foreign born)
- Visa (H1B, TN, O1)
- Naturalization Certificate
- Valid Permanent Resident Card
- Valid Employment Authorization Card and Valid Passport (Foreign)
