Many states require an NPDB/HIPDB (National Practitioner Data Bank/Healthcare Integrity and Protection Data Bank) as part of their application process. It is easy to prepare an NPDB/HIPDB request online. Here is a quick link to the NPDB/HIPDB’s website Click on “Perform a Self Query”, read the Rules of Behavior and check the box at the bottom. Click “Continue” then click “Individual Self Query”. Enter your information including all state licenses you’ve ever held whether they be active, inactive or expired. Include your credit card information on the request form. The NPDB report costs $8 and the HIPDB report costs $8 for a total of $16. When finished entering your information, click “Continue”. Click “Continue” again and verify your information If any corrections need to be made, click the browser back button and make the corrections. (DO NOT CLICK THE “RETURN TO OPTIONS” BUTTON. IT WILL DELETE ALL OF YOUR INFORMATION). Your signature on this form must be notarized so do not sign it until you are in the presence of a Notary Public. Your bank should have a notary on staff. If not, you can search Google to find a notary in your area. Mail the notarized form to the NPDB/HIPDB. Both their P.O. Box and their physical address is listed on the first page of the form. Both reports will be returned to you in one envelope at the address you enter on the form. DO NOT OPEN THE REPORT!!! Many medical boards will not accept the report if it has been opened. Mail the unopened report to the medical board when you receive it.
